First appointment checklist

1. We will be sending you an email to set up your account and fill out new patient paperwork online. If you do not get a chance to do it, please plan to arrive 10-20 minutes EARLY to fill out your intake form and paperwork in person.

2. Wear loose-fitting clothing that allows for easy access to elbows and knees, as well as your area of concern. 

 

3. Don’t come on an empty or full stomach, but a small snack is okay. Keep in mind you may be lying facedown on your stomach for an extended period of time.

 

4. What to bring: government-issued ID, insurance card, medical records (optional), list of medications (optional) 

 

5. Make a list of your top concerns and be ready to discuss them with us!
